Class Participation *

I understand that I must show up to all training classes without fail or I will be removed from the program without refund. This includes vacations.
I understand that I must put in 100% effort as an owner and follow all directions from the trainers and participate in DAILY video check ins

All dogs are required to be up to date on the following vaccines. Please provide the expiration date for each vaccine’s *

I confirm that my dog is up to date on Bordatella, DHPP, and Rabies vaccine.

Client Expectations: Check to acknowledge *

Our training program involves the use of a Control Collar aka “choke collar”. This training tool is used in conjunction with proper hands on training that you are about to receive. Your trainer will go over proper use when returning for your first lesson to ensure proper use of control collar
Follow all instructions given and record yourself in daily accountability check-in videos every week and send directly to your trainer except on days with your trainer.
I as the client agree to meet with my trainer on a weekly basis and follow training program without modification and will provide weekly progress videos as part of my training plan homework.
As the client I am participating in this program with All American K9 Academy Inc freely and willingly and will not hold All American K9 Academy responsible for any theft, damages and or injuries that may occur during my training lessons with All American K9 Academy Inc regardless of training location including but not limited to clients home, public locations and our training centers.

I hereby acknowledge that I am voluntarily participating in Dog training classes with All American K9 Academy Inc. All American K9 Academy Inc. will exercise all necessary and proper care and precautions to keep all participants and dogs safe during training sessions in order to prevent injury to any person, property, or dog. However, because dogs are animals and may be unpredictable, All American K9 Academy Inc. cannot protect against every potential injury, risk or accident. This Release and Waiver are intended to shift the risk of participating in dog training to the Participant/Owner/Guardian of the dog participating in training. I acknowledge that I have had full opportunity to discuss any and all concerns I have regarding foregoing risks with All American K9 Academy Inc. and its authorized representatives. Participant agrees to hold harmless All American K9 Academy Inc. from any and all claims of injury, loss, cost, expenses, or damage caused by the actions of Participant's dog during the course of training, or under the care, custody or control of All American K9 Academy Inc. I hereby accept and assume, without reservation, all risks associated with my participation in the training program, including, but not limited to: the risks of any and all injuries to myself, my dog and any of my family members or third parties who may attend; the risks that my dog may cause injury to other persons and/or dogs involved; and the risks that saliva, water, food, and/or other debris may be present in the training area. I have read and understand All American K9 Academy Inc. policies and abide by them.As lawful consideration for participating in the training program, I, for myself, my heirs, next of kin, executors, administrators, legal representatives, successors and assigns (the “Releasing Party”), hereby waive, release, discharge and covenants not to sue and to indemnify, defend and hold harmless, All American K9 Academy Inc. and their members, managers, agents, and employees (the “Released Party”) from any and all injuries, losses, claims and damages to any person or persons of any nature its directors, officers, employees, agents, sponsors, organizers, volunteers or other representatives, their successors and/or assigns, (hereinafter collectively the “Releases”) from any and all liability to the Undersigned for any loss, harm, damage, claim, injury or accident to the person or property of the Undersigned, whether caused by any ordinary negligent act or omission of the Releases while the Undersigned or his/her dog is engaged in or participating in dog training/boarding or using any Trainer or Business facilities in connection therewith.This Waiver of Liability & Informed Consent Release shall be legally binding on the Releasing Party. Further, Should the Releasing Party assert a claim to the contrary to what I have agreed to in this Waiver of Liability and Informed Consent Release, the claiming party shall be liable for all expenses (including attorney’s fees, court costs and consultant fees) incurred by both the Releasing Party and the Released Party. No waiver or modification of any provision herein shall be valid unless expressly agreed to in writing by both the Released Party and the Releasing Party.I hereby consent to my dog being the subject of the photographs and/or audiovisual recording and authorize All American K9 Academy Inc. to cause the same to be exhibited for educational purposes or marketing tools as still photographs, video or other similar media. NOTICE TO BUYER: Promotional examination fees, equipment (i.e. training gear, additional uniforms and tournament fees are NOT INCLUDED in the program pricing. *

Refund Policy: Check to acknowledge *

Once training has started All American K9 Academy Inc cannot provide any refund of any kind. If program cancelled prior to first lesson we will refund your entire tuition cost however evaluation fee cannot be refunded. Our trainers reserve the right to refuse services or dismiss client from program if client is not following training plan including but not limited to repeat cancellation of appointments, not completing homework assignments, unsafe behavior and any behavior that is inappropriate.
I understand fully what is expected of me and will follow all instructions given to me by my trainer. I agree to the above and will communicate with my trainer when needed with questions or concerns and hold myself accountable for the outcome of my training I am enrolling my dog in.

I hereby understand that there are four classes that cannot be skipped unless an emergency or accepted by head trainer. If I skip a class I acknowledge and accept that I may be removed from class without refund. *

I understand full and accept terms
